MCT and Exosmart, Explained
MCT stimulates. Exosmart concentrates. They are not rivals — and understanding the difference is the best way to understand how modern regenerative trichology actually works.

Since we introduced Exosmart™ autologous exosome therapy at the clinic, one question has come up in consultation more than any other: "What is the difference between this and MCT? And which one is better?"
It is an excellent question — and the honest answer surprises most patients: they are not competitors, and "better" is the wrong frame. MCT and Exosmart™ do fundamentally different jobs with the same raw material. Understanding that difference is the best way to understand how modern regenerative trichology actually works.
1What they have in common
Both technologies are autologous — meaning everything used in your treatment comes from your own blood, drawn in clinic during your visit. No donor material, no cultured cells, no plant-derived substitutes. Both begin the same way: a standard blood sample, separated by centrifugation into plasma.
And both exist for the same underlying reason: conventional platelet-rich plasma, valuable as it is, leaves much of your blood's regenerative potential untapped. Where they part ways is in what happens to that plasma next.
Both MCT and Exosmart™ are fully autologous — your own blood, separated by centrifuge in clinic, used entirely for your own treatment.
2MCT: conditioning your biology to perform
MCT — Meta Cell Technology — is a photothermal biomodulation system. Your prepared plasma is placed in a certified closed cassette and exposed to specific light wavelengths together with precisely controlled temperature for around ten to fifteen minutes.
The principle comes from photobiomodulation science: light at particular wavelengths interacts with cellular components — notably the mitochondria — while the gentle thermal stimulus encourages your platelets to release more of their growth factors and signalling vesicles, in a sustained way rather than a single burst.
A useful way to picture it: MCT is a training session for your plasma. The volume does not change, the composition is not filtered — but the cells within it are conditioned to give more.
The manufacturer is explicit on this point: MCT does not concentrate anything. It optimises. The same preparation you would have used for conventional PRP performs differently after photothermal conditioning — not because the quantity of material has changed, but because the biological readiness of that material has been enhanced.
3Exosmart™: gathering more of your biology
Exosmart™ approaches the same plasma from the opposite direction. Rather than conditioning it, it physically refines it — passing the full plasma volume through an ultrafiltration membrane with pores smaller than five nanometres. Only water and salts pass through; everything biologically active is retained and concentrated into a small final volume.
Critically, this includes the fraction that standard PRP protocols discard — which is where most of your exosomes, being far smaller than platelets, actually end up. The final preparation is your complete secretome, concentrated: exosomes, platelets, growth factors and structural plasma proteins together.
If MCT is a training session, Exosmart™ is a harvest. Less volume, far denser in signalling material.
Exosmart™ ultrafiltration retains everything biologically active from your plasma — including the exosome fraction that standard PRP preparation discards.
4Stimulation and concentration: side by side
Once the mechanisms are clear, the relationship between the two becomes obvious. One improves how your biology performs. The other gathers more of it. These are not overlapping functions — they are sequential logics, acting on different levers of the same regenerative process.

5Why they complement each other
This is why the question "which is better?" has no universal answer, and why in our practice the two are positioned as complementary tools within a personalised protocol rather than rival products on a menu. For one patient, photothermal conditioning of a standard preparation may be the appropriate step. For another, a concentrated autologous preparation may be indicated. For some, a protocol may draw on both — sequenced, not stacked arbitrarily — where clinical assessment supports it.
What decides between these paths is not preference, price or novelty. It is diagnosis.

6An honest word about evidence
As with everything we publish, candour matters. The performance figures associated with both technologies originate primarily from manufacturer studies rather than large independent peer-reviewed trials. Both devices are CE-certified with well-documented engineering and safety profiles; the independent clinical evidence base in hair applications is still maturing across the entire regenerative category.
This is not a reason to dismiss these tools. It is a reason to use them the way we do: selected on the basis of instrumental diagnosis, and monitored objectively over time — so your results are measured on your scalp, not assumed from a brochure.
7It starts under the trichoscope
Every regenerative decision at the clinic begins with trichoscopy: magnified, structured examination of your scalp and follicles. The trichoscopic picture determines whether a regenerative protocol is appropriate for you at all, which approach fits your situation, where on the scalp it should be targeted, and — at every follow-up — whether it is doing its job.
So when patients ask me "MCT or Exosmart?", my answer is always the same: let's look first. The right technology is a conclusion, not a starting point.
